1. Complete and return this application to the Director of Advancement of Bethany School. This information will be held in strictest confidence.

2. After receiving the application and application fee, Bethany School will contact you regarding testing and the school visit.

3. A $50.00 fee must be included with this application. This is applied against the cost of processing and is non-refundable.
If submitting the application online, you must mail in your application fee as we do not accept online payments.
The fee and the application will remain active through the child's 8th grade year. Checks may be made to Bethany School.

4. Kindergarten students must be five by September 30.

I understand that if an enrollment contract is issued, a deposit of $200.00 must be submitted to hold the child's place. This non-refundable deposit is
applied toward the fees for the school year. I understand that if the child is accepted and enrolled, the school reserves the right to dismiss any student
under the following circumstances: if, in the school's judgement, the family and school are not able to work together in the best interest of the child; if the
family fails to maintain the financial commitment; or if the student does not abide by the school's behavioral expectations. Bethany School admits students
without regard to race, creed, color, or national origin.
